Everyone has to fill out an online form sooner or later. Practice by filling out this simple example form.

TIPS:

1. Capitalize the first letters of your name. Use the SHIFT key for capitals. Never use CAPS LOCK.
2. After completing the First Name field, tap the TAB key on your keyboard to advance to the next field. Hold down the SHIFT key and tap the TAB key to move backwards.
3. Fields marked with an asterisk ( * ) are required. The submit button will not work if any of these fields are empty.
4. In the State field, click the field and select the state from the list.
5. In the Date of Birth Field, enter the month, day and year directly, or click the icon to use the calendar.
6. You can only choose one Gender (radio buttons) but you can select as many Favorite Colors (check boxes) as you want .
7. The Choose File button simulates uploading a file. It will open your File Explorer. Double click on a file to select it.
8. The information is not sent anywhere when you click submit. Practice as much as you like.
